ONLINE ELIGIBILITY SCREENING CONSENT 
 
Title of Research Study: Effects of Trauma, Discrimination, and Stress on the Daily Lives of Transgender and Gender Diverse People

Thank you for your interest in our research. The purpose of this research study is to
understand the effects of exposure to stressful experiences on bodily responses, emotions,
behavior, and well-being. This survey will ask you a few questions to determine whether
you are eligible to participate in this research.
 
The screening will take about 5 minutes to complete. You will be asked some questions
about yourself, including demographic questions and questions about your experiences.
You do not have to answer any questions you do not wish to answer or are uncomfortable
answering, and you may stop at any time. Your participation in the screening is voluntary.
We will make our best efforts to keep your answers confidential. No one except for the
research team will have access to your answers. If you do not qualify for the study, your
responses will not be stored. If you qualify for the study and decide to participate, your
screening responses will be kept with the research record.
