ONLINE ELIGIBILITY SCREENING CONSENT 
 
Title of Research Study: Self-Perception and Intimacy after the Cancer Experience
Principal Investigator: Megan Wirtz
 

Thank you for your interest in participating in our research. You are being asked to participate in this study because you are between the ages of 18-39, have a history of cancer, and have completed cancer treatment at least six months ago. The purpose of this research study is to find out more about life after cancer, specifically to understand the impact of cancer sexual wellbeing components, including sexual health knowledge, interpersonal relationships, and body image among young adult survivors.

 

This survey will ask you a few questions to determine whether you are eligible to participate in this research.

The screening will take about 3 minutes to complete. You will need to complete a CAPTCHA and then will be asked some questions about yourself, such as demographics, your cancer history, and access to internet. You do not have to answer any questions you do not wish to answer or are uncomfortable answering, and you may stop at any time. However, completion of the CAPTCHA is required, as well as uploading relevant contact information so that we may reach out if you are eligible. Your participation in the screening is voluntary. We will make our best efforts to keep your answers confidential. No one except for the research team will have access to your answers. If you do not qualify for the study, your responses will not be stored. If you qualify for the study and decide to participate, your screening responses will be kept with the research record.     

 

If you are determined to be eligible to participate in the study, you will receive an email from our research team with next steps. 

 

Participation in this study will involve:

- An online questionnaire, which is estimated to take 60-70 minutes

- Compensation of $15 total in the form of an Amazon Gift Card following completion of the online survey.
